Job Summary

As a member of this team, you will play a vital role in providing psychosocial assessments and care management services within the Geriatrics Center's Neurology services (in particular the movement disorders and cognitive disorders programs) and Primary Care clinic to facilitate an understanding

and resolution of problems related to the management of acute geriatric and chronic neurological diseases, as well as environmental needs, interpersonal conflict, and physical, emotional, mental health and substance use concerns. You will be able to analyze, compare, and evaluate various courses of action with the authority to make independent recommendations on matters of significance, within the scope of your responsibilities.

Responsibilities*
You will be responsible for:
  • Collaborating with Geriatrics Center Health Care Teams for ongoing patient care and coordination
  • Conducting patient psycho-social and family assessments
  • Identifying needs of patients and families for ongoing health care and social service support
  • Assisting patients/families with transitions of care, such as outpatient, inpatient, sub-acute (rehab) and to other medical services and treatments.
  • Assuming responsibility for difficult and complex cases based on experience or are of expertise.
  • Working directly with patients and families both face-to-face and by phone contacts.
  • Brief therapeutic interventions drawing on evidence-based psychotherapies, psychoeducation, safety planning, assisting family members in understanding and supporting the patient, grief counseling, and referrals to community resources are some of the skills needed and additional duties of this position.

Required Qualifications*
  • A Master's degree in Social Work from a school accredited by the Council of Social Work Education.
  • A current license to practice social work in the State of Michigan.

Desired Qualifications*
  • Previous experience with medical and mental health social work
  • Previous experience with neurological diseases related to movement disorders (particularly Parkinson's Disease, Huntington's Disease, Ataxia, and Functional Movement Disorders) and cognitive disorders (particularly Alzheimer's Disease, Lewy Body Dementia, Frontotemporal dementia, and other dementia conditions)
  • Certificate or significant course work in aging studies
  • Skilled in developing rapport and effectively communicating with a diverse patient/family population
  • Efficient, accurate assessment of patient and family needs and effective case dispositions
  • Effective organizational skills, flexibility, self-motivation, the ability to effectively prioritize multiple competing tasks in a dynamic medical clinic
  • Independent, sound decision-making is necessary for success in this position
  • Effective collaboration with a large multi-disciplinary team
  • Proficiency in keeping accurate and timely clinical records by computer
  • Experience with evidence-based psychotherapeutic interventions and familiarity with brief, structured interventions (e.g., Motivational Interviewing, Behavioral Activation) along with a basic knowledge of psychopharmacology

Work Schedule
This is full time, 40 hour/week position. The typical schedule is Monday-Friday with start and end times 8:00-4:30 or 8:30-5:00.

Underfill Statement
This position may be underfilled at a lower classification depending on the qualifications of the selected candidate. Advancement through the career ladder at the Expert or Clinical Scholar Level is required for placement in the MSW Senior Classification

Background Screening
