Overview:
The Student Affairs Clinical Case Manager is under the direction of the Associate Vice President of Student Health, Counseling and Wellness and the general direction of the Vice President for Student Affairs. This role provides case management services to students experiencing varying degrees of need and/or distress.

The Clinical Case Manager assists students in optimizing their educational experience while minimizing the impact of illness, injury, behavioral changes, basic needs and personal distress on students' academic careers; coordinates care, interventions, social support with appropriate referrals, and follow-up services within a broad network of campus and community resources; provides a safe, welcoming, and affirming environment for all students; provides case management and support to students in crisis, students with complex psycho-social needs and students in need of support related to food, housing and financial insecurities; collaborates in providing support to students in distress by triaging concerns and coordinating a response from the campus community.

The incumbent is readily accessible and provides consultation to faculty, staff, students, parents and family members who are concerned about a distressed Fresno State Student. The Clinical Case Manager works closely with key offices (University Police, Counseling and Psychological Services, Academic Affairs, Equal Opportunity and Employee Relations, Student Conduct, CARE team, Campus Response Team and Support Net) to identify and intervene as early as possible with students of concern. The Clinical Case Manager also functions as an intermediary in a collaborative care team that includes the patient's primary care provider, a psychiatric consultant, and other mental health providers in the Student Health & Counseling Center, to provide enhanced and integrated mental health care to patients. Clinical case managers serve as a resource to primary care providers, coordinate referrals, provide patient education, support treatment development, adherence and revision, promote behavior change, and help track treatment outcomes.

Position Summary:
Major Duties of the Job Include:
  • Provides coordination and linkage to community services for eligible students; maintains a list of available resources and working relationships with those resources
  • Provides or facilitates in-clinic or outside referrals to evidence-based psychosocial treatments (e.g, CBT, IPT) as clinically indicated and to other supportive services as appropriate
  • Participates as a back-up in the Care Team, including attending weekly meetings as needed, making clinical judgments that may often be complex, and responding to students' needs as appropriate
  • Participates on other committees as appropriate
  • Assists with the Good Samaritan Grant program
  • Assists with the emergency housing program and meal voucher distribution
  • Participates on other committees (as required) dealing with campus safety; presents information or provides trainings to various groups on campus or in the community
  • Assists with The Body Project, including developing materials, training student staff, making necessary arrangements and assisting with marketing
  • Provides counseling as part of the integrated care model
    o Coordinates consultation by a mental health specialist on patients new to treatment or who are not improving as expected; communicates resulting treatment recommendations to the patient's PCP
    o Facilitates communication between patient's PCP and mental health provider to improve coordination of services and information flow
    o Works with patients' providers to customize services to best meet patients' needs
    o Identifies provider issues and recommendations for improvement
  • Assists in screening and assessing patients for common mental health and substance abuse disorders
  • Interprets and evaluates descriptions and explanations of problems brought forward by a student, student organizations, faculty, or staff; analyzes and defines the problem, draws valid conclusions, and projects consequences of various alternative courses of action
  • Provides patient education about common mental health and substance abuse disorders and available treatment options; addresses patient concerns, clarifies treatment preferences, and reviews and explains treatment goals and expectations; completes relapse prevention plan with patients who are in remission
  • Provides psychoeducation, focusing on self-management, utilizing methods such as Behavioral Activation, problem-solving, Motivational Interviewing, and other evidence-based techniques
  • Supports psychotropic medication management prescribed by PCPs, focusing on treatment adherence, side effects and other complications, and effectiveness of treatment
  • Facilitates patient engagement and follow-up (in person or by telephone) to monitor changes in clinical symptoms, treatment adherence, treatment side effects or complications; addresses any patient concerns
  • Monitors and tracks treatment outcomes in the EMR system using standard assessments and symptom scales (PHQ9); uses the EMR system to document in-person and telephone encounters and to identify and re-engage patients; identifies and reports PI clinical, utilization and outcomes issues
  • Facilitates treatment plan changes for patients who are not improving as expected, which may include changes in medications or psychosocial treatments or appropriate referrals for additional services
  • Documents patient progress and treatment recommendations in the EMR system registry so they can be easily shared with PCPs, the psychiatric consultant, and other treating providers
  • Effectively presents information and responds to questions from various groups including managers, faculty, staff, students, media, and the community
  • Assists in the preparation of departmental, division, and university reports
  • Assists with maintaining a web presence and utilizing various marketing strategies

    Secondary Duties of the Job Include:

    Knowledge, Skills & Abilities:
    Knowledge of:
  • Policies, procedures and practices related to providing services for students in a Health and Wellness Center or the ability to quickly acquire such knowledge
  • General policies, practices and activities related to behavioral health education, community organizations and Student Services programs outside the program to which immediately assigned
  • General principles, problems and methods of public administration, including organizational, personnel and fiscal management
  • General knowledge of advanced statistical and research methods
  • Principles, theories, techniques and practices of clinical social work and various therapeutic, treatment and recovery approaches, including individual, group and family counseling
  • Laws, regulations and ethical standards governing behavioral health treatment and medical records
  • Behavioral health interviewing techniques, treatment methods, theories and causes of mental health and substance use disorders
  • Computer office systems with the ability to use a broad range of technology, systems, and software packages (Microsoft Office Suite, Google, Excel and PowerPoint)

    Education and Experience:
    Education:
    Equivalent to graduation from a four-year college or university in a related field, including or supplemented by upper division or graduate course work in counseling techniques, interviewing and conflict resolution where such are job related.

    Experience:
    Possession of these knowledge and abilities is typically demonstrated through the equivalent to three years of progressively responsible professional student services work experience.

    A master's degree in Counseling, Clinical Psychology, Social Work or a job-related field may be substituted for one year of professional experience. A doctorate degree and the appropriate internship or clinical training in counseling, guidance or a job-related field may be substituted for three years of experience.

    Preferred Skills:
    Preferred Knowledge, Skills and Abilities:
  • Licensed as a clinical social worker
  • A considerable degree of tact and persuasiveness to achieve the desired results of understanding and/or cooperation
  • Experience with screening for common mental health and/or substance use disorders
  • Experience with assessment and treatment planning for common mental health and/or substance use disorders
  • Experience with linking patients to appropriate community resources
  • Working knowledge of differential diagnosis of common mental health and/or substance use disorders
  • Working knowledge of evidence-based psychosocial treatments for common mental health disorders
  • Familiarity with brief, structured intervention techniques (e.g., Motivational Interviewing, Behavioral Activation)
  • Basic knowledge of psychopharmacology for common mental health disorders that is within appropriate scope of practice for type of provider filling role
