SUMMARY:
The Case Manager provides counseling services to assist students in maximizing their potential for personal and academic success. Exhibit a commitment to a developmental model by initiating and implementing interventions that promote a healthy campus environment and meet the mental health needs of the University community.

DUTIES & RESPONSIBILITIES:
  • Provide clinical case management services including assisting students with locating resources to address basic and mental health needs (e.g. financial strains, food insecurity, access to healthcare).
  • Serve as community referral coordinator. Maintain and update referral and resource database. Establish relationships with key referral sources and maintain knowledge of current wait times, scope of services, and other parameters affecting referral options. Advise clinicians about available campus and community referral options. Meet with clients to arrange referrals and assist student in navigating the process. Manage clients being discharged to Counseling Center from inpatient treatment.
  • Provide intensive case management and transition services (bridge care) for students who periodically need additional services to address risk and also to students transitioning to community providers or specialists. Consult with Counseling Center Nurse Practitioner to address prescription related questions from clients.
  • Assist with triage counselor-on-duty services as assigned. Participate in weekly case conferences and informal case consultation to coordinate client care. Obtain professional consultation when appropriate. Inform team of high-risk students and student of concern.
  • Provide clinical supervision to practicum trainees and interns.
  • Provide consultation and outreach services to students, campus and community members related to mental health issues and student concerns.
  • Assist the Director with updates to electronic health record system templates and running electronic health record system reports.
  • Perform other duties as assigned.

MINIMUM REQUIREMENTS:
  • Education: Master's degree in applicable field or equivalent combination of education and experience.
  • Experience: Five (5) years of experience in counseling or related field. One (1) year (post-graduate) experience in social work, counseling, or other mental health agency. Experience providing direct clinical case management services, crisis services, triage, care coordination, and referrals to adolescents and adults.
  • Knowledge / Skills: Understanding of relevant intervention models, programs and resources. Knowledge of the mental health needs of a university population. Multicultural competence and experience working effectively with individuals representing a wide range of diverse communities and backgrounds. Excellent organizational and administrative abilities required to effectively document in an Electronic Health Record system, and maintain a case management tracking system. Strong work ethic and sense of collegiality. Knowledge of word processing, spreadsheet, and database applications. Knowledge of student development. Excellent verbal and written communication skills. Excellent interpersonal skills.
  • Ability to: Ability to multitask and work cooperatively with others. Ability to present information clearly and concisely. Familiar with confidentiality requirements for case managers within counseling center agency.
  • Licensing/Professional Certifications: Licensed as a licensed clinical social worker (LCSW), licensed master social worker (LMSW), or licensed professional counselor (LPC) in the State of Texas or able to obtain within 6 months of employment.

  • Other Requirements: Work beyond normal office hours and/or work on weekends.

PREFERRED EDUCATION / SKILLS/ EXPERIENCE:
Eight to ten (8 to 10) years (post-graduate) experience in social work, counseling, or other mental health agency. Case management experience in college mental health setting. Currently licensed in the State of Texas. Training and experience in brief therapy and crisis intervention with adults in an outpatient setting. Experience providing clinical supervision to master's level counseling and psychology trainees. Live within 30 minutes of campus (Commerce).

SUPERVISION OF OTHERS:
Graduate assistants, interns, and practicum students.
